LEGAL NOTICE
Pursuant to F.S. 98.075(7), notice is hereby given to the voters listed below. Please be advised that your eligibility to vote is in question based on information provided by the State of Florida or the Clerk of the Circuit Court. You are required to contact the Supervisor of Elections in Volusia County, Florida, no later than 30 days after the date of this notice to receive information regarding the basis for the potential ineligibility and the procedure to resolve the matter.  This includes the right to request a hearing for the purpose of providing information to substantiate your status.  Failure to respond will result in a determination of ineligibility by the Supervisor of Elections and your name will be removed from the Florida Voter Registration System. 

If you attempt to vote at an early voting site or your normal election day polling place, you will be required to vote a provisional ballot.  If you vote by mail, your ballot will be treated as a provisional ballot. In either case, your ballot may not be counted until a final determination of eligibility is made.  If you wish for your ballot to be counted, you must contact the Supervisor of Elections in Volusia County within two days after the election and present evidence that you are eligible to vote.  If you have any questions pertaining to this matter, please contact the Supervisor of Elections at 1588 N Woodland Blvd., DeLand, Florida or call 386-736-5930.
